At Beauty Mojo, we exclusively utilize superior-grade products, eyelash extensions and adhesive for our treatment procedures. Though most clients do not have any reaction / discomfort during and post procedure, the sensitivity and reactions to products may vary among individuals. In the unlikely event of encountering significant discomfort or an allergic response during the session, I will promptly notify the attending lash technician. Subsequently, the technician will undertake the removal process at no charge, and I shall seek medical advice at my own expense.

It is advised that individuals who are pregnant / nursing / with medical conditions seek the authorization of their physician prior to scheduling an eyelash extension appointment. Beauty Mojo shall not be held liable for any complications arising during or subsequent to the treatment.

It is not recommended to undergo eyelash extension procedures within six months following any surgical intervention in the vicinity of the eye, including permanent makeup. Prior to obtaining eyelash extensions, I shall obtain consent from my physician. Beauty Mojo shall not assume responsibility for any resulting repercussions.

I acknowledge that in order to undergo the application of eyelash extensions, it is necessary for me to recline and keep my eyes shut for a duration of 60 to 180 minutes. Any medical conditions that could be exacerbated by prolonged periods of immobility may mean that I will not be a suitable candidate for eyelash extensions.

I understand and agree to keep my eyes closed throughout the entire eyelash extension procedure. I acknowledge that excessive movement or touching my eyes without the technician's guidance may result in the displacement of tapes and eye patches, potentially causing corneal abrasion.

I hereby absolve the technician performing the eyelash extension procedure of any responsibility for outcomes associated with this procedure. I recognize that, although maximum effort is applied to ensure the longevity of eyelash extensions, the persistence of the bond cannot be universally guaranteed due to individual variability and external factors. We consistently aim for optimal retention; however, on rare occasions, unforeseen variances in product manufacturing may impact the durability of the eyelash extensions' bond. Should I experience any concerns, such as a notable loss of extensions within the initial days following the procedure, I commit to promptly contacting the technician for support. It is crucial that I am available to come down for a consultation or correction within the specified early follow-up window of three days after the procedure. I am aware that Beauty Mojo is not obliged to address any complaints regarding bonding issues if reported beyond this period.

This agreement shall endure for the current and any subsequent procedures administered by my designated technician. I acknowledge the obligatory nature of this agreement and affirm that I have comprehensively reviewed and comprehended all the information presented herein.
